There is nothing on this card to tell who the model is, nor what company sent out the card, just the 1-800 number to call to claim a "free" product.  Sounds fishy to me, but after ten or fifteen years, i don't suppose it matters  (for those who need these things spelled out, this is a card from somewhere around the turn of this century, and I'm selling only that.  That's not my phone number on the back, and I never did find out whose it was.  JUST selling the card.)
